Execution of pharmaceutical interventions by an experience in clinical pharmacy

Abstract

Objective: Describing pharmaceutical's performance in clinical practice by analysis of accomplished interventions. Methods: An observational and retrospective study was done in a private institution in Salvador – Bahia in a period from 2011 to 2014. This Hospital serves adult patients, with a capacity for 60 beds, focusing on Cardiology, Pneumology, Neurology and Oncology especialities. Medical prescriptions are made through a computer-based system by the physicians. Hospital's medicine distribution system is the individually dose for 24 hours. Pharmaceutical interventions were done through participation of this professional in multidisciplinary approaches, evaluation of medical prescriptions and solicitations of the multidisciplinary team. These pharmaceutical interventions were classified according to standardized categories in the institution. Results: 2346 interventions were performed from 2011 to 2014, wherein by the year of the service implementation, in 2012, 206 interventions were done. In 2013, there were 925 interventions and in 2014, there were 1215 interventions. Medication reconciliation was the most frequent intervention in 2012 (20%), requirement of additional treatment in 2013 (24%) and necessity of changing drug scheduling in 2014 (19%). The acceptability of performed interventions during this period was 88%. Conclusion: The increasing number of performed interventions and its consequent acceptability along the years ratifies the importance of the clinical pharmacist, since his role in the multidisciplinary team aims to promote quality for the patient's therapy, through medication errors prevention and promotion of safe and rational use of medicines.
